Our Care team ensures the creation and maintenance of healthy and rewarding relationships with caregivers and members/clients.
Caregivers are able to receive remuneration every month, plus room and board per member/client, depending on the level of care.
We provide a wealth of information to caregivers to assist them in ensuring that the individual needs of our members/clients are met.

Our mission is to provide physician ordered Adult Foster Care Services to members in a qualified setting, assisting them to achieve the highest level of potential in their day- to-day activities. Member can stay home happily, healthy, and safely with family members/care giver for longest period without any involvement with nursing homes or long-term hospital stays.
The Adult Foster Care Program (AFC), a Medicaid/MassHealth funded program, pays family and non-family caregivers for their oversight and clinical assistance of eligible adults with physical, cognitive, or medical disabilities who cannot live safely alone.

Assisting Individuals, Families and Caregivers
AFC recipients must require assistance
in at least one of the following areas: mobility in or outside their home, bathing, dressing, toileting, eating or transferring. A registered nurse and care manager will conduct an assessment to determine eligibility. The AFC Program provides financial reimbursement to family members/caregivers. Services are provided
24 hours a day, 7 days a week by the family member/ caregiver to the AFC recipient living in their home.
Anyone can refer an eligible adult who can’t live safely alone or needs clinical assistance. Referrals can come from family members, private parties, public agencies, physicians, nursing homes or
hospitals.

Parents who are not legal guardians of their 18 and older child, other family members, and friends
Spouses and parents who are legal guardians cannot be Caregiver.
Child is over 18 and the parents are co- guardians, one parent can receive the stipend if the other relinquishes guardianship or another person becomes the guardian
Need to be approved through AFC provider home-study process as caregivers
