Adult Foster Care is a federally funded MassHealth program that is designed to provide living alternatives to individuals who are elderly or disabled. These individuals usually require assistance while performing activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
ADLs include activities such as eating, toileting, dressing, bathing, transferring, and walking. IADLs are activities related to independent living that are incidental to an individual’s care, such as household management, laundry, shopping, housekeeping, meal preparation, transportation, and medication monitoring.
As a licensed Adult Foster Care program, SAHAJ Adult Foster Care provides industry-specific training to caregivers which assist them in delivering care in their homes.
We have a team of expert registered nurses and Case Managers who carry out regular home visits, monitor members' health needs, and develop personalized care plans.
